Haploidentical donor regulatory T cells + memory T cells is an investigational adoptive cell therapy strategy in which a hematopoietic stem cell transplant graft from a haploidentical (half-matched) related donor is engineered or supplemented to contain enriched populations of donor-derived regulatory T cells (Tregs) and antigen-experienced memory T cells, typically in the setting of haploidentical allogeneic hematopoietic cell transplantation for high‑risk hematologic malignancies such as acute leukemias and myelodysplastic syndromes.[2][7] Donor Tregs are intended to limit graft‑versus‑host disease and enable reduced or potentially absent pharmacologic immunosuppression, while donor memory T cells (often including tumor‑ or pathogen‑reactive subsets) are infused to provide rapid immune reconstitution, graft‑versus‑leukemia activity, and improved infection control after transplant.[2][7] This approach remains in early‑phase academic clinical development and is delivered as a bespoke cellular product manufactured from each individual donor leukapheresis rather than a standardized commercial drug.[2][7]
