Haploidentical hematopoietic stem cells (KIR-HLA selected) refers to an allogeneic cell therapy protocol developed by Memorial Sloan Kettering Cancer Center for the treatment of high-risk hematologic malignancies. This intervention involves the transplantation of unmodified bone marrow-derived hematopoietic stem cells from a partially HLA-mismatched (haploidentical) related donor. The core of the therapy is a novel donor selection algorithm that prioritizes donors based on specific Killer Immunoglobulin-like Receptor (KIR) and Human Leukocyte Antigen (HLA) genetic profiles. This selection is designed to maximize the alloreactivity of donor-derived natural killer (NK) cells against the recipient's malignant cells, thereby enhancing the graft-versus-leukemia (GVL) effect. The protocol is typically administered following a conditioning regimen of melphalan, fludarabine, and thiotepa, and is being evaluated for various conditions including acute myeloid leukemia (AML), acute lymphoid leukemia (ALL), and myelodysplastic syndromes (MDS).
