Haploidentical hematopoietic stem cells combined with umbilical cord blood (often referred to as a 'haplo-cord' transplant) is an investigational cellular therapy approach developed at Weill Medical College of Cornell University. The procedure involves the co-infusion of CD34+ selected progenitor cells from a partially matched (haploidentical) related donor alongside a unit of umbilical cord blood. This approach is designed to overcome the slow engraftment typically associated with cord blood transplants by using the haploidentical cells for rapid initial neutrophil and platelet recovery, while the cord blood provides durable long-term hematopoiesis and a potent graft-versus-leukemia effect. It is primarily indicated for patients with high-risk hematologic malignancies, such as acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS), who lack a human leukocyte antigen (HLA)-identical donor.
