The combination of HCG (human chorionic gonadotropin), FSH (follicle-stimulating hormone), and zinc is used primarily to treat male infertility and hypogonadism. Zinc serves as a complementary nutritional element that plays a vital role in reproductive processes. Studies have shown that zinc supplementation can significantly increase semen volume, sperm motility, and normal sperm morphology, especially when combined with folate[8]. ## Treatment Protocol The combination therapy typically begins with HCG administration alone. After several months, if adequate serum testosterone levels are achieved but no sperm is detected, FSH treatment is introduced[1]. The duration of treatment may vary from 6-24 months or longer, continuing until sperm appears in the ejaculate and/or pregnancy is achieved[1]. Most studies have shown that gonadotropins (HCG + FSH) induce spermatogenesis in approximately 80% of treated men, with some demonstrating success rates up to 94%[1]. The average time for sperm to appear in the ejaculate is about seven months, with an average time of 28 months to achieve pregnancy[1]. ## Efficacy and Outcomes The pregnancy rate varies between studies, with 38% to 51% of couples achieving pregnancy with this treatment approach[1]. The therapy also increases testosterone levels and testicular volume with relatively few side effects[1]. Several factors affect treatment response. Negative prognostic indicators include cryptorchidism, small testicles, elevated BMI, and extreme gonadotropin insufficiency[1]. ## Side Effects Common side effects of HCG may include: - Injection site pain - Headache - Fatigue - Depression - Mood swings - Acne - Weight gain - Breast enlargement[5][7] FSH injections may cause: - Injection site pain - Headache - Fatigue[2] The combination therapy is generally well-tolerated, and proper dose adjustments to optimize testosterone levels will minimize side effects[1].
