HDM2005 + lenalidomide is an investigational combination regimen being evaluated for the treatment of B-cell malignancies, particularly relapsed or refractory mantle cell lymphoma (MCL). HDM2005 is a Class 1 innovative antibody-drug conjugate (ADC) targeting receptor tyrosine kinase-like orphan receptor 1 (ROR1). It is composed of a humanized IgG1 monoclonal antibody conjugated to the microtubule-disrupting cytotoxic payload monomethyl auristatin E (MMAE) via a proteolytically cleavable linker. Lenalidomide is an oral immunomodulatory drug (IMiD) that binds to the cereblon (CRBN) E3 ubiquitin ligase complex, leading to the degradation of transcription factors such as Ikaros (IKZF1) and Aiolos (IKZF3), which results in anti-proliferative and pro-apoptotic effects. The combination, often administered with rituximab, aims to provide synergistic anti-tumor activity by combining targeted cytotoxicity with immunomodulation.
