Overview

This combination therapy involves the sequential or concurrent use of photodynamic therapy (PDT) with hematoporphyrin (or its derivative, HpD) and immunotherapy with Bacillus Calmette-Guérin (BCG). It is primarily investigated for the treatment of non-muscle invasive bladder cancer (NMIBC). Hematoporphyrin acts as a photosensitizing agent that, upon activation by specific wavelengths of light, generates reactive oxygen species (ROS) to induce direct tumor cell death and vascular damage. BCG is a live-attenuated strain of Mycobacterium bovis that, when instilled intravesically, induces a robust local inflammatory and immune response in the bladder wall, leading to the destruction of residual cancer cells. The combination aims to leverage the immediate cytotoxic effects of PDT with the sustained immunological surveillance provided by BCG to enhance therapeutic efficacy and reduce recurrence rates. Research on this combination has been notably conducted by institutions such as the Cancer Hospital of the Chinese Academy of Medical Sciences and Shenzhen Hospital.
