Henagliflozin + probiotics is a combination therapy being investigated for the treatment of heart failure with preserved ejection fraction (HFpEF), particularly in elderly patients. The combination consists of henagliflozin (SHR3824), a potent and selective sodium-glucose cotransporter 2 (SGLT2) inhibitor developed by Jiangsu Hengrui Pharmaceuticals, and a probiotic preparation (clostridial enterococcus triple live tablets). Henagliflozin works by inhibiting glucose and sodium reabsorption in the proximal tubule of the kidney, leading to osmotic diuresis and natriuresis, which reduces cardiac preload and afterload and improves myocardial energy efficiency. The probiotic component aims to modulate the gut microbiota, enhance intestinal barrier function, and reduce systemic inflammatory markers, addressing the intestinal dysbiosis often observed in heart failure patients. This dual approach is hypothesized to provide synergistic benefits in cardiac and renal function, exercise tolerance, and quality of life.
