Heparin + certoparin is a combination of two anticoagulant drugs used for the prevention and treatment of venous thromboembolic events. Heparin (specifically, unfractionated heparin or UFH) is a naturally occurring anticoagulant that works by enhancing the activity of antithrombin III, thereby inhibiting thrombin and factor Xa, which are key enzymes in the blood coagulation cascade. Certoparin is a low molecular weight heparin (LMWH) derived from standard heparin through depolymerization; it primarily inhibits factor Xa via antithrombin III with less effect on thrombin compared to UFH. Both agents are administered parenterally and are indicated for thromboprophylaxis in acutely ill patients at risk for deep vein thrombosis or pulmonary embolism. Clinical studies have shown that certoparin is non-inferior to unfractionated heparin in preventing venous thromboembolism among elderly, acutely ill non-surgical patients, with similar or lower rates of bleeding and adverse events[1].
