Overview

This is a combination regimen of three drugs—heparin (an anticoagulant), clopidogrel (a P2Y12 receptor inhibitor antiplatelet agent), and aspirin (an antiplatelet agent)—used for the prevention and treatment of thrombotic cardiovascular events. Heparin acts by potentiating the activity of antithrombin III to inhibit clotting factors IIa (thrombin) and Xa, reducing blood coagulation. Clopidogrel irreversibly inhibits the platelet P2Y12 ADP receptor, preventing platelet aggregation. Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) in platelets, blocking thromboxane A2 formation and thus inhibiting platelet activation. This combination is used in acute coronary syndromes or after percutaneous coronary intervention to reduce ischemic complications but carries an increased risk of major bleeding compared to single or dual therapy[1][2][5].
