Heparin + lidocaine + adrenaline is a non-standardized multi-drug combination typically prepared extemporaneously, combining the anticoagulant and glycosaminoglycan heparin with the amide local anesthetic lidocaine and the vasoconstrictor catecholamine adrenaline (epinephrine). Heparin acts as a glycosaminoglycan layer enhancer and anticoagulant, potentially reinforcing mucosal barrier function and reducing transepithelial solute migration, while lidocaine blocks voltage-gated sodium channels in sensory nerves to provide local anesthesia and additional anti-inflammatory and mast cell–modulating effects. Adrenaline produces local vasoconstriction via alpha-adrenergic receptor activation, which can reduce systemic absorption of lidocaine and prolong its local anesthetic effect, as demonstrated for lidocaine–adrenaline solutions in surgical anesthesia. The exact clinical use and dosing of this triple combination depend on local protocols (e.g., pain control and mucosal protection in urologic or procedural settings) and it is not a widely standardized or branded pharmaceutical product.
