Overview

Heparin + salbutamol is a combination therapy involving the co-administration of nebulized heparin and salbutamol (also known as albuterol). Heparin is an anticoagulant that, beyond its blood-thinning properties, exhibits anti-inflammatory effects and can reduce the adherence of bacteria and viruses to bronchial surfaces. Salbutamol is a short-acting beta-2 adrenergic agonist bronchodilator that relaxes airway smooth muscle, improving airflow in conditions like asthma and chronic obstructive pulmonary disease (COPD). The combination has been studied primarily for use in mechanically ventilated patients with acute exacerbations of COPD (AECOPD), where it has shown efficacy in increasing ventilator-free days without increasing bleeding risk. Heparin acts by potentiating antithrombin III activity to inhibit clotting factors, while salbutamol stimulates beta-2 adrenoceptors to induce bronchodilation and also exerts anti-inflammatory effects by inhibiting mast cell degranulation and phospholipase A2 activity[1][5][7].
