Overview

Heparin + tranexamic acid is a combination therapy consisting of two pharmacologically distinct agents used together in specific clinical scenarios, particularly for the management of bleeding disorders where both excessive coagulation and fibrinolysis are present. Heparin is an anticoagulant that potentiates the activity of antithrombin III, thereby inhibiting thrombin and factor Xa to prevent clot formation. Tranexamic acid is an antifibrinolytic agent that competitively inhibits activation of plasminogen to plasmin, preventing fibrin degradation and stabilizing clots. This combination has been reported in case studies for conditions such as disseminated intravascular coagulation (DIC) with prominent hyperfibrinolysis, where controlling both clot formation and breakdown is necessary[1][2][7]. The use of this combination requires careful monitoring due to potential risks associated with altering both coagulation and fibrinolytic pathways.
