This is a combination regimen consisting of four agents used in the management and prevention of hepatitis B virus (HBV) infection, particularly in high-risk or treatment-resistant scenarios. - **Hepatitis B Immune Globulin (HBIG)** is a human polyclonal antibody preparation that provides passive immunity by supplying anti-HBs antibodies, preventing HBV infection or reinfection, especially after exposure or liver transplantation[6][8]. - **Entecavir** is a nucleoside analogue reverse transcriptase inhibitor that blocks HBV DNA polymerase activity, inhibiting viral replication[5][7]. - **Tenofovir disoproxil** and **tenofovir alafenamide** are both prodrugs of tenofovir; they are nucleotide analogue reverse transcriptase inhibitors that inhibit HBV polymerase by competing with natural substrates, leading to chain termination during viral DNA synthesis. Tenofovir alafenamide achieves higher intracellular concentrations at lower doses compared to tenofovir disoproxil. This combination may be considered for patients with multidrug-resistant HBV or those undergoing liver transplantation to prevent reinfection and achieve maximal viral suppression[1][2][3].
