HER-Vaxx + chemotherapy is a combination therapy consisting of **HER-Vaxx**, a B-cell peptide vaccine targeting the HER2/neu receptor, administered together with standard-of-care chemotherapy regimens for patients with HER2-overexpressing advanced or metastatic gastric or gastroesophageal junction adenocarcinoma. HER-Vaxx (IMU-131) consists of three B-cell epitopes derived from the extracellular domain of HER2, conjugated to the carrier protein CRM197 and formulated with the adjuvant Montanide. The vaccine triggers the patient's immune system to produce HER2-specific polyclonal antibodies, promoting anti-tumor effects via antibody-dependent cell cytotoxicity (ADCC) and inhibition of intracellular HER2 phosphorylation. When combined with chemotherapy (typically cisplatin plus 5-fluorouracil or capecitabine, or oxaliplatin plus capecitabine), the combination significantly improves overall survival compared to chemotherapy alone in HER2+ metastatic/advanced gastric or GEJ adenocarcinoma, as shown in phase II clinical trials. HER-Vaxx is under development by Imugene and intended as an alternative or complement to monoclonal antibody therapies like trastuzumab for HER2-positive cancers[1][2][3][4][6].
