Hetrombopag + dexamethasone is a combination therapy being investigated for the treatment of primary immune thrombocytopenia (ITP). Hetrombopag is a non-peptide, small-molecule thrombopoietin receptor (TPO-R) agonist developed by Jiangsu Hengrui Pharmaceuticals. It works by binding to the transmembrane domain of the TPO receptor on megakaryocytes, stimulating their proliferation and differentiation to increase platelet production. Dexamethasone is a potent synthetic glucocorticoid that provides rapid immunosuppression, reducing the clearance of platelets by the reticuloendothelial system and inhibiting the production of anti-platelet antibodies. The combination is designed to provide a synergistic effect by simultaneously increasing platelet production and decreasing platelet destruction, aiming for higher response rates and sustained remission compared to monotherapy. Clinical development for this combination is notably being led by institutions such as The First Affiliated Hospital of Xi'an Jiaotong University.
