Overview

This high-dose multivitamin intervention was specifically developed for the Folic Acid for Vascular Outcome Reduction in Transplantation (FAVORIT) trial, a large-scale study sponsored by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK). The formulation consists of high doses of folic acid (5.0 mg), vitamin B6 (50 mg), and vitamin B12 (1.0 mg), along with standard doses of other B vitamins and vitamin C. Its primary mechanism of action is the reduction of total homocysteine (tHcy) levels by providing essential cofactors for the remethylation and transsulfuration pathways of homocysteine metabolism. The intervention was targeted at stable kidney transplant recipients who frequently exhibit hyperhomocysteinemia, a condition associated with increased risk for arteriosclerotic cardiovascular disease. Although the multivitamin effectively lowered homocysteine levels in the trial population, the study concluded that it did not significantly reduce the incidence of cardiovascular disease events, leading to the discontinuation of this specific high-dose regimen for this indication.
