This is an adoptive cellular immunotherapy developed by the National Institutes of Health Clinical Center (CC) for the treatment of HIV infection in progressors who have failed standard antiretroviral therapy. The therapy involves the infusion of HLA-B*57-positive peripheral blood lymphocytes collected via apheresis from 'elite controllers'—rare individuals who naturally suppress HIV replication without medication. The mechanism of action relies on the transfer of functional CD8+ cytotoxic T lymphocytes (CTLs) that specifically recognize and eliminate HIV-infected cells through peptides restricted by the protective HLA-B*57 allele. The Phase 1 study evaluates the safety, immunologic effects, and potential antiviral activity of this cell-based biologic approach in recipients who share the HLA-B*2705 or HLA-B*57 protective alleles.
