A combination therapy of highly purified human menopausal gonadotropin (hMG-HP) and recombinant follicle-stimulating hormone (r-FSH) is used in controlled ovarian hyperstimulation protocols for assisted reproductive technologies such as in vitro fertilization (IVF). hMG-HP contains both FSH and LH activity, with the LH activity primarily provided by human chorionic gonadotropin (hCG), while r-FSH is a recombinant form of FSH with minimal or no LH activity. The combination aims to leverage the benefits of both agents—FSH for stimulating follicular growth and LH/hCG for supporting steroidogenesis and final oocyte maturation. Clinical studies suggest that this combination may improve implantation rates, clinical pregnancy rates, embryo quality, and reduce the risk of ovarian hyperstimulation syndrome compared to monotherapy[1][2][6]. The mechanism involves direct stimulation of ovarian follicles via FSH receptor agonism by both components; hMG-HP also provides additional LH/hCG receptor agonism.
