hMN-14 x m734 + indium-111-labeled IMP-205 is a two-step pretargeting radioimmunodetection and radioimmunotherapy system developed for the management of carcinoembryonic antigen (CEA)-producing tumors, specifically colorectal cancer. The system utilizes a bispecific antibody (hMN-14 x m734) composed of two Fab' fragments: a humanized fragment (hMN-14) that targets CEA and a murine fragment (m734) that targets a chelated metal or peptide moiety. In clinical application, such as the PRISCO trial, the bispecific antibody is administered first to localize at the tumor site. After the antibody has cleared from the circulation, an Indium-111 labeled peptide (IMP-205), which contains histamine-succinyl-glycine (HSG) or DTPA moieties, is administered. This peptide binds rapidly to the pre-localized bispecific antibody, allowing for high-sensitivity nuclear imaging (radioimmunodetection) or targeted delivery of therapeutic radionuclides with reduced systemic toxicity compared to directly labeled antibodies.
