HPN-07 + N-acetylcysteine

**HPN-07 + N-acetylcysteine** is an investigational combination therapy consisting of HPN-07 (disodium 2,4-disulfophenyl-N-tert-butylnitrone), a free radical spin trapping agent, and N-acetylcysteine (NAC), a glutathione precursor and antioxidant. The combination acts synergistically to reduce oxidative stress, neuroinflammation, apoptosis, ischemia-reperfusion injury, and neurodegeneration, particularly in the auditory pathway. It has been studied primarily for preventing and treating noise-induced hearing loss (NIHL), blast-induced auditory injury, and tinnitus, with preclinical evidence of protecting cochlear hair cells, ribbon synapses, and spiral ganglion neurons, as well as restoring auditory brainstem response thresholds. A phase 1 trial (NCT02259595) assessed safety, tolerability, and pharmacokinetics of oral HPN-07 alone and with NAC in healthy volunteers. Developers include Oklahoma Medical Research Foundation and related investigators; no marketing approval yet.
