Overview
HQK-1004 + valganciclovir is an investigational combination therapy evaluated primarily for the treatment of Epstein-Barr virus (EBV)-positive lymphoid malignancies or lymphoproliferative disorders. HQK-1004, also known as arginine butyrate or VX-105, is a small molecule agent designed to modulate cellular targets—specifically, it acts as a modulator (stimulant) of thymidine kinase 1 (TK1), with preclinical and clinical studies exploring its role in cancer and hematologic diseases[1][3][7]. Valganciclovir is an antiviral prodrug of ganciclovir, primarily approved and marketed as Valcyte, used for the treatment and prevention of cytomegalovirus (CMV) infections in immunocompromised populations; its mechanism of action involves inhibition of viral DNA polymerase following phosphorylation and incorporation into viral DNA, ultimately terminating DNA chain elongation[2][4][6]. The rationale for combination in EBV-driven malignancies is to enhance viral antigen expression (via HQK-1004) and enable the cytotoxic activity of valganciclovir/ganciclovir, targeting EBV-infected tumor cells[5].
