HRS-4642 + nab-paclitaxel + gemcitabine + nimotuzumab is an investigational combination therapy being evaluated for the treatment of borderline resectable pancreatic cancer with KRAS G12D mutations. The regimen includes HRS-4642, a selective, non-covalent, liposome-formulated small molecule inhibitor of the KRAS G12D mutant protein; nimotuzumab, a humanized monoclonal antibody targeting the epidermal growth factor receptor (EGFR); and the chemotherapy doublet AG (nab-paclitaxel and gemcitabine). HRS-4642 suppresses downstream MEK and ERK phosphorylation, while nimotuzumab blocks EGFR-mediated signaling. The combination aims to provide a multi-pronged attack on pancreatic ductal adenocarcinoma (PDAC) by targeting both the primary driver mutation and complementary growth pathways. This therapy is currently in Phase II clinical development led by Zhejiang University.
