Huang bai pellets + S-1 is an investigational combination therapy being evaluated for the treatment of advanced pancreatic cancer, specifically pancreatic ductal adenocarcinoma (PDAC). This regimen combines Huang Bai pellets, a Traditional Chinese Medicine (TCM) formulation derived from the bark of *Phellodendron chinense* or *Phellodendron amurense*, with S-1, a standard oral fluoropyrimidine chemotherapy agent. S-1 is a multi-component drug consisting of tegafur (a prodrug of 5-fluorouracil), gimeracil (a dihydropyrimidine dehydrogenase inhibitor that maintains high 5-FU levels), and oteracil (which reduces gastrointestinal toxicity). Huang Bai is traditionally used to clear heat and detoxify, and in the context of oncology, it is hypothesized to exert anti-tumor effects by inhibiting inflammatory pathways such as NF-κB and modulating the tumor microenvironment. The combination is being developed by the Institute of Basic Research in Clinical Medicine at the China Academy of Chinese Medical Sciences to enhance the efficacy of chemotherapy while potentially mitigating its adverse effects.
