Overview

Human umbilical cord mesenchymal stem cell-derived exosomes (hUCMSC-Exos) are extracellular vesicles secreted by mesenchymal stem cells (MSCs) harvested from human umbilical cord tissue. These nano-sized vesicles contain a complex cargo of bioactive molecules, including proteins, lipids, and various RNA species (miRNA, mRNA), which mediate paracrine signaling and immunomodulatory effects. In the treatment of non-acute immune checkpoint inhibitor-related pneumonitis (CIP), hUCMSC-Exos are delivered via nebulization to the lungs. The therapeutic mechanism involves suppressing excessive pulmonary inflammation, promoting tissue repair, and modulating the local immune environment by regulating cytokine levels (such as IL-1β, IL-6, and IL-10) and immune cell subsets like regulatory T cells (Tregs). This cell-free approach aims to provide the regenerative benefits of MSC therapy while potentially reducing risks associated with live cell administration.
