Overview

Hydralazine + nesiritide is a combination of two agents used in cardiovascular medicine. Hydralazine is a small molecule vasodilator and antihypertensive agent indicated for the management of essential hypertension and as adjunct therapy in heart failure. It acts primarily by directly relaxing arteriolar smooth muscle through mechanisms that are not fully understood but involve interference with calcium metabolism within vascular smooth muscle cells[2][5]. Nesiritide is a recombinant form of human B-type natriuretic peptide (hBNP), produced using recombinant DNA technology in E. coli[6][8]. It binds to the particulate guanylate cyclase receptor (NPR-A) on vascular smooth muscle and endothelial cells, increasing intracellular cGMP levels and causing vasodilation, natriuresis, diuresis, and inhibition of the renin-angiotensin-aldosterone system[1][4][6][8]. Nesiritide is approved for intravenous use in acutely decompensated congestive heart failure with dyspnea at rest or minimal exertion.
