Overview

Hydrochlorothiazide + potassium is a combination therapy consisting of the thiazide diuretic hydrochlorothiazide and an electrolyte supplement, potassium (typically administered as potassium chloride). Hydrochlorothiazide acts by inhibiting the sodium-chloride symporter (SLC12A3) in the distal convoluted tubule of the nephron, which increases the urinary excretion of sodium and water, thereby reducing blood volume and blood pressure. Because thiazide diuretics also promote the excretion of potassium, leading to potential hypokalemia, potassium is co-administered to maintain electrolyte homeostasis. In the context of research at Brigham and Women's Hospital, this combination was used as an active control in a Phase 2 clinical trial (NCT00100178) to evaluate the effects of mineralocorticoid receptor blockade on vascular function and injury in patients with Type 2 Diabetes Mellitus.
