Hydrocortisone + ascorbic acid + thiamine, often referred to as HAT therapy or the Marik protocol, is a combination treatment investigated for the management of sepsis and septic shock. The therapy utilizes three components with synergistic mechanisms: hydrocortisone, a synthetic glucocorticoid that reduces systemic inflammation and restores vascular responsiveness to vasopressors; ascorbic acid (Vitamin C), a potent antioxidant that protects the endothelial barrier and works synergistically with corticosteroids; and thiamine (Vitamin B1), a crucial cofactor for the enzyme pyruvate dehydrogenase that facilitates aerobic metabolism and helps reduce lactate levels. This specific intervention was evaluated in a prospective randomized double-blinded controlled trial by Tanta University to assess its impact on mortality and organ dysfunction in critically ill patients in the surgical intensive care unit (SICU).
