Hydrocortisone + lidocaine is a combination therapy consisting of a synthetic glucocorticoid corticosteroid and a local anesthetic of the amino amide type. Hydrocortisone acts as an anti-inflammatory agent by binding to and activating glucocorticoid receptors, which leads to the suppression of pro-inflammatory cytokines and the modulation of the immune response. Lidocaine provides rapid, temporary analgesia by inhibiting voltage-gated sodium channels, thereby preventing the initiation and conduction of nerve impulses. In clinical research conducted by the National University Hospital, Singapore, this combination (often utilizing triamcinolone as the steroid component in practice) is evaluated for the treatment of De Quervain's tenosynovitis. The mixture is typically administered via intrathecal injection into the first extensor compartment of the wrist to alleviate pain and reduce the stenosing inflammation of the tendon sheaths.
