**Hydrocortisone + neomycin sulfate + bacitracin zinc + polymyxin B sulfate** is a sterile prescription ophthalmic ointment combining a glucocorticoid anti-inflammatory agent with three topical antibacterial agents. Hydrocortisone activates the glucocorticoid receptor to suppress ocular inflammation; neomycin inhibits bacterial protein synthesis through the 30S ribosomal subunit; bacitracin disrupts bacterial cell-wall precursor recycling; and polymyxin B binds lipopolysaccharide and disrupts Gram-negative bacterial membranes. It is indicated for steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment when bacterial infection is present or a meaningful infection risk exists, including chronic anterior uveitis and corneal injury from chemical, thermal, or radiation burns or foreign bodies. The combination is contraindicated in most viral corneal or conjunctival diseases, mycobacterial ocular infection, and fungal ocular disease.
