Scarguard (also known as Scarguard ScarCare) is a topical over-the-counter combination product containing 0.5% hydrocortisone, 12% silicone gel, and vitamin E. Developed by Red Rock Laboratories and clinically investigated by the University of Miami, it is designed for the treatment and prevention of keloids and hypertrophic scars. The formulation is a clear brush-on liquid that rapidly dries to form a flexible, nearly invisible protective nanofilm. Its therapeutic effect is achieved through a multi-pronged mechanism: hydrocortisone acts as a glucocorticoid receptor agonist to inhibit fibroblast proliferation and collagen synthesis; the silicone film provides occlusion that stimulates collagenase production to break down excess scar tissue; and vitamin E further aids in reducing collagen synthesis. Scarguard has been evaluated in Phase 4 clinical trials comparing its efficacy and safety against other topical treatments like onion extract gel.
