Drug intelligence / Profile preview
Hydromorphone is a potent semi-synthetic opioid analgesic and a hydrogenated ketone of morphine. The Dilaudid CR formulation, developed by Alza Corporation, utilizes the proprietary OROS (Osmotic Release Oral System) technology to provide controlled, extended release of the active ingredient over a 24-hour period. This delivery system is designed to maintain stable plasma levels of hydromorphone, thereby providing continuous pain relief for patients with chronic conditions such as osteoarthritis of the hip or knee. Its primary mechanism of action involves agonism of the mu-opioid receptors in the central nervous system, which inhibits the ascending pain pathways and modulates the emotional response to pain. While originally developed as Dilaudid CR, this specific OROS formulation was later approved and marketed under the brand names Exalgo in the United States and Jurnista in other international markets.
