Overview

Hydromorphone + morphine is a combination of two potent opioid analgesics used for the management of moderate to severe pain. Both drugs act primarily as agonists at the mu-opioid receptor, leading to analgesia, sedation, and euphoria. Hydromorphone is significantly more potent than morphine—approximately 2–8 times greater by some estimates and up to 20 times more potent parenterally—requiring careful dose adjustment when switching between these agents[2][3]. Morphine serves as the standard comparator for opioid efficacy in clinical practice[3][4]. Both drugs can cause respiratory depression, constipation, nausea, and have a high potential for abuse and dependence[1][5]. They are typically reserved for cases where non-opioid analgesics are insufficient.
