Overview

Hydroxychloroquine and azithromycin is a combination therapy involving two distinct pharmaceutical agents. Hydroxychloroquine is an antimalarial and immunomodulatory drug, commonly used in the treatment of autoimmune diseases such as rheumatoid arthritis and lupus. Azithromycin is a macrolide antibiotic primarily indicated for bacterial infections. The combination has been investigated, particularly during the COVID-19 pandemic, for potential antiviral effects against SARS-CoV-2, though evidence remains controversial[2][5]. Mechanistically, hydroxychloroquine interferes with lysosomal activity and autophagy in host cells, modulates immune responses by inhibiting toll-like receptor signaling, and may alter endosomal pH required for virus/cell fusion. Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit but also exhibits immunomodulatory properties[6]. Proposed mechanisms for their combined antiviral action include interference with viral entry or replication via complex formation with magnesium ions that target viral RNA structures[6]. However, this combination carries significant risk of cardiac toxicity (notably QT interval prolongation), especially when used together or in patients with pre-existing cardiovascular risk factors[1][3][4][8].
