Overview

Hydroxychloroquine + dexamethasone is a combination therapeutic regimen being investigated for the treatment of antinuclear antibody (ANA)-positive primary immune thrombocytopenia (ITP). Hydroxychloroquine is an established antimalarial and immunomodulatory small molecule that inhibits lysosomal activity and Toll-like receptor (TLR) signaling, particularly TLR7 and TLR9. Dexamethasone is a potent synthetic glucocorticoid that provides broad immunosuppression by activating the glucocorticoid receptor. This specific combination is being evaluated in clinical trials led by researcher Yunfeng Cheng at Zhongshan Hospital, Fudan University, to determine if the addition of hydroxychloroquine to standard dexamethasone therapy improves platelet response rates and duration in adult ITP patients who test positive for ANA, a subpopulation often associated with refractory disease.
