This is a combination of three agents—hydroxychloroquine, potassium chloride, and sodium bicarbonate. Hydroxychloroquine is an antimalarial and disease-modifying anti-rheumatic drug (DMARD) used for malaria prophylaxis and treatment, as well as autoimmune diseases like rheumatoid arthritis and lupus erythematosus. It acts by interfering with lysosomal activity and autophagy in immune cells, modulating the immune response. Potassium chloride is an electrolyte supplement used to treat or prevent hypokalemia (low blood potassium), essential for normal cell function. Sodium bicarbonate is an alkalinizing agent used to correct metabolic acidosis or to alkalinize urine; it can also be administered in certain drug intoxications due to its ability to counteract sodium channel blockade-induced QRS widening on ECG. There are no known approved pharmaceutical products that combine these three agents into a single formulation; however, this combination may be encountered in clinical practice during the management of hydroxychloroquine overdose or toxicity. In such cases, potassium chloride corrects hypokalemia induced by hydroxychloroquine toxicity (which causes intracellular shift of potassium), while sodium bicarbonate may be administered if there is QRS widening due to sodium channel blockade[1]. Each component has distinct mechanisms of action: - Hydroxychloroquine blocks voltage-gated sodium and potassium channels at toxic doses. - Potassium chloride replenishes serum potassium. - Sodium bicarbonate increases serum pH and provides extracellular sodium ions that help reverse cardiac conduction abnormalities caused by hydroxychloroquine overdose[1][2].
