Overview

Hydroxychloroquine + valsartan is a combination therapy investigated for the treatment of primary IgA nephropathy (IgAN). Hydroxychloroquine is an antimalarial and immunomodulatory small molecule that reduces immune activation by inhibiting Toll-like receptor 9 (TLR9) signaling and the subsequent production of inflammatory cytokines, including interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and interferon-alpha (IFN-α). Valsartan is an angiotensin II receptor blocker (ARB) that provides renoprotection by antagonizing the angiotensin II type 1 (AT1) receptor, leading to reduced blood pressure and decreased proteinuria. This combination was specifically evaluated by the Peking Union Medical College Hospital in the HCQIgAN trial to address persistent proteinuria in IgAN patients who have an inadequate response to standard ARB therapy alone.
