Hydroxyethyl starch + sodium chloride is an intravenous solution used as a plasma volume expander for the treatment and prophylaxis of hypovolemia (low blood volume) due to trauma, surgery, or other causes. It consists of hydroxyethyl starch (HES), a synthetic colloid derived from amylopectin, dissolved in isotonic saline (0.9% sodium chloride). HES acts by increasing the oncotic pressure within blood vessels, thereby expanding plasma volume and maintaining hemodynamic stability after significant fluid loss[1][2][3]. The mechanism involves reducing capillary leakage and retaining fluid intravascularly for several hours after administration[1]. However, use is associated with increased risks of acute kidney injury, coagulopathy (bleeding disorders), and mortality—especially in critically ill patients or those with pre-existing renal dysfunction[5][8]. Due to these safety concerns, regulatory agencies recommend its use only when adequate alternative treatments are unavailable.
