Hyper-CVAD + maintenance chemotherapy (Peking University protocol) is an intensive multi-agent chemotherapy regimen used for the treatment of adult patients with standard-risk acute lymphoblastic leukemia (ALL) in first complete remission (CR1). Developed and evaluated by researchers at Peking University, this protocol serves as a consolidation and maintenance strategy. The consolidation phase consists of alternating cycles of Hyper-CVAD-A (cyclophosphamide, vincristine, doxorubicin, and dexamethasone) and Hyper-CVAD-B (high-dose methotrexate and cytarabine). This is followed by a long-term maintenance phase comprising methotrexate, 6-mercaptopurine, and monthly pulses of vindesine and prednisone (VP). The regimen employs a diverse array of mechanisms, including DNA alkylation, inhibition of DNA and RNA synthesis, tubulin polymerization inhibition, and glucocorticoid-mediated apoptosis, to eradicate residual leukemic cells and prevent relapse.
