Hyperbaric oxygen + dexamethasone is a combination therapeutic regimen investigated for the treatment of brain radionecrosis, a late complication of radiation therapy characterized by tissue death and inflammation. The regimen, notably studied in clinical trials led by the Barrett Cancer Center (University of Cincinnati), utilizes hyperbaric oxygen therapy (HBOT) to deliver 100% oxygen at pressures exceeding one atmosphere. This hyperoxygenation is intended to reverse chronic hypoxia, stimulate angiogenesis, and promote the healing of radiation-damaged microvasculature. Dexamethasone, a potent synthetic glucocorticoid, is administered concurrently to manage the inflammatory response and reduce perifocal cerebral edema (swelling) associated with necrotic lesions. This dual approach aims to stabilize neurological symptoms and promote tissue recovery in patients who have failed standard steroid therapy alone.
