Hypertonic saline + dextran-70 (HSD) is a combination intravenous solution used primarily for small-volume resuscitation in cases of hypovolemic shock due to trauma or burns. The formulation typically consists of 7.5% sodium chloride (hypertonic saline) and 6% dextran-70 (a high-molecular-weight polysaccharide). Hypertonic saline rapidly expands plasma volume by drawing water from the interstitial and intracellular spaces into the intravascular compartment, thereby increasing blood pressure and cardiac output[2][5][8]. Dextran-70 acts as a plasma expander by increasing oncotic pressure, prolonging the intravascular effect of the hypertonic solution[7]. This combination has been shown to restore hemodynamics more effectively than isotonic fluids in some settings and may modulate immune responses by blunting neutrophil activation and enhancing T cell function[8]. However, its benefits on long-term survival remain inconclusive based on meta-analysis data[1].
