Overview

This therapeutic regimen, investigated by Barnes-Jewish Hospital, involves the sequential administration of intravenous hypertonic saline (2.8% or 4.4% NaCl) followed by oral sodium chloride supplementation for the treatment of decompensated congestive heart failure (CHF) in patients with renal dysfunction. The treatment is designed to be administered simultaneously with loop diuretics like furosemide. Mechanistically, the hypertonic saline increases intravascular osmotic pressure, which facilitates the mobilization of fluid from the interstitial space into the vasculature. This helps maintain renal perfusion and prevents the worsening of kidney function (prerenal physiology) that often occurs during aggressive diuresis. The subsequent oral sodium chloride supplementation, combined with a normosodic diet, aims to sustain this effect post-acute treatment, potentially reducing hospital readmission rates and mortality in severe CHF populations.
