Overview

Ianalumab + corticosteroids is a combination therapy under investigation by Novartis for the treatment of primary immune thrombocytopenia (ITP). Ianalumab (VAY736) is a fully human, afucosylated monoclonal antibody targeting the B-cell activating factor receptor (BAFF-R). It works through a dual mechanism: depleting B cells via enhanced antibody-dependent cellular cytotoxicity (ADCC) and blocking BAFF-R-mediated survival signals. Corticosteroids, the standard-of-care first-line treatment for ITP, act as broad immunosuppressants by binding to the glucocorticoid receptor. The combination is currently being evaluated in the Phase III VAYHIT1 trial for newly diagnosed adult patients with primary ITP to achieve sustained platelet responses and reduce the need for subsequent lines of therapy.
