Overview

Ibalizumab + antiretrovirals is a combination drug regimen used for the treatment of multidrug-resistant HIV-1 infection in heavily treatment-experienced adults who are failing their current antiretroviral regimen[1][3][5]. Ibalizumab is a humanized monoclonal antibody that targets CD4 T cell receptors, functioning as a CD4-directed post-attachment HIV-1 inhibitor. Unlike other entry inhibitors, ibalizumab binds to domain 2 (and partially to domain 1) of the CD4 extracellular region, inducing conformational changes that prevent the HIV envelope glycoprotein gp120 from interacting with co-receptors CCR5 and CXCR4, thereby blocking subsequent viral fusion and entry[2][4][6]. This mechanism is effective against both CXCR4- and CCR5-tropic HIV-1 strains. The regimen should be administered intravenously and always in combination with an optimized background antiretroviral therapy, and is reserved for patients with documented resistance to at least one antiretroviral from each of three classes (NRTI, NNRTI, PI)[5]. The combination has demonstrated efficacy in reducing viral load and improving immunological outcomes without significant immunosuppression or major adverse effects.
