Overview

Ibalizumab + chidamide is an investigational combination regimen being evaluated for the treatment of HIV-1 infection, specifically targeting the reduction of the latent HIV reservoir. This therapeutic approach utilizes a 'shock and kill' strategy: chidamide (also known as tucidinostat) acts as a selective histone deacetylase (HDAC) inhibitor and latency reversal agent to reactivate ('shock') latent HIV-1 from cellular reservoirs, while ibalizumab, a humanized IgG4 monoclonal antibody, acts as a post-attachment inhibitor by binding to domain 2 of the CD4 receptor to block viral entry and facilitate the clearance ('kill') of reactivated cells. The combination is currently being studied in a Phase 1 dose-escalation clinical trial led by the First Affiliated Hospital of Zhejiang University in adults living with HIV who are on stable antiretroviral therapy (ART).
