Iberdomide (CC-220) is a potent, next-generation Cereblon E3 ligase modulator (CELMoD) developed by Bristol Myers Squibb (formerly Celgene). It is designed to bind to cereblon (CRBN), a component of the CRL4-CRBN E3 ubiquitin ligase complex, thereby inducing the recruitment and subsequent proteasomal degradation of the lymphoid transcription factors Ikaros (IKZF1) and Aiolos (IKZF3). This mechanism leads to direct tumoricidal effects and immune-stimulatory activity, including the activation of T cells and natural killer (NK) cells. Iberdomide is primarily being investigated for the treatment of hematologic malignancies, most notably relapsed or refractory multiple myeloma, where it has shown clinical activity both as a monotherapy and in combination with other agents like dexamethasone, daratumumab, or bortezomib. It is also being explored for systemic lupus erythematosus (SLE) due to its ability to modulate B-cell and T-cell function.
