Iberogast (STW 5) and Iberogast N (STW 5-II) are multi-component herbal medicinal products developed for the treatment of functional gastrointestinal disorders, including functional dyspepsia and irritable bowel syndrome (IBS). Iberogast is a liquid formulation of nine herbal extracts, while Iberogast N is a modified formulation containing six of those extracts, notably excluding celandine, milk thistle, and angelica root. These preparations exert their therapeutic effects through a multi-target mechanism, including the modulation of gastrointestinal motility via serotonin (5-HT3 and 5-HT4) and muscarinic (M3) receptors, reduction of visceral hypersensitivity, and anti-inflammatory activities. Originally developed by Steigerwald Arzneimittelwerk (now part of Bayer), these products have a long history of clinical use and have been evaluated in Phase 4 trials, such as those conducted by Hospital Vall d'Hebron, to assess their impact on intestinal gas transit and abdominal symptoms.
