Overview

IBI188 + granulocyte macrophage colony-stimulating factor is a combination therapy under development, primarily evaluated in clinical settings for the treatment of newly diagnosed higher-risk myelodysplastic syndrome (MDS). **IBI188** is a recombinant human IgG4 anti-CD47 monoclonal antibody. Its mechanism of action involves blocking CD47, a "don't eat me" signal, on tumor cells, thereby promoting their clearance via macrophage-mediated phagocytosis. Granulocyte macrophage colony-stimulating factor (GM-CSF) is a cytokine used to stimulate the production and function of granulocytes and macrophages, potentially enhancing antitumor immunity when combined with agents such as IBI188. The combination seeks to synergistically improve immune-mediated eradication of malignant cells by enhancing both innate immune activation (via GM-CSF) and the removal of immune-evasive signals (via CD47 blockade)[1]. IBI188 is developed by Innovent Biologics for oncology indications, notably MDS. Clinical trials primarily evaluate the IBI188 + azacitidine combination, but the mechanism supports the potential for combinations with immune stimulants like GM-CSF. The main indication is higher-risk MDS, an aggressive blood disorder with limited therapeutic options and poor prognosis. Combination immunotherapies targeting macrophage activation are an active research area in hematologic malignancies.
