IBI343 + sintilimab + oxaliplatin + S-1 is a multi-modality combination therapy regimen being developed by Innovent Biologics for the perioperative treatment of claudin 18.2 (CLDN18.2)-positive, resectable, locally advanced gastric or gastroesophageal junction adenocarcinoma. This regimen integrates IBI343, a recombinant humanized anti-CLDN18.2 antibody-drug conjugate (ADC) conjugated to an exatecan payload (a topoisomerase I inhibitor) via a cleavable linker, with sintilimab, a monoclonal antibody targeting programmed cell death protein 1 (PD-1). The combination also includes the SOX chemotherapy regimen, which consists of the platinum-based DNA cross-linker oxaliplatin and the oral fluoropyrimidine antimetabolite S-1. The therapeutic strategy aims to leverage the targeted cytotoxicity of the ADC, the immune checkpoint inhibition of sintilimab, and the systemic effects of chemotherapy to improve pathological complete response rates and survival outcomes in patients with CLDN18.2-expressing tumors.
