Overview

**Ibrexafungerp + dabigatran** is a combination of two distinct pharmaceutical agents: ibrexafungerp, a first-in-class oral triterpenoid antifungal agent, and dabigatran, a direct oral anticoagulant (DOAC), specifically a direct thrombin inhibitor. **Ibrexafungerp** inhibits fungal β-1,3-D-glucan synthase, a key enzyme responsible for fungal cell wall biosynthesis, leading to cell lysis. It is active against *Candida* and *Aspergillus* species and is FDA-approved for the treatment and prevention of vulvovaginal candidiasis, including cases intolerant or resistant to standard azole or echinocandin therapy[1][2][5][6][8]. Developed as an oral alternative to intravenous echinocandins, it is designed to address resistance and improve patient convenience[4]. **Dabigatran** inhibits thrombin (Factor IIa), preventing the conversion of fibrinogen to fibrin and thus inhibiting coagulation. It is approved for stroke prevention in atrial fibrillation, treatment and prevention of venous thromboembolism, and other indications.
