Overview

A combination of two oral small-molecule anticancer agents used together investigationally for B‑cell lymphomas. Ibrutinib is a covalent inhibitor of Bruton’s tyrosine kinase that blocks B‑cell receptor signaling, leading to decreased proliferation and survival of malignant B cells. Lenalidomide is an immunomodulatory agent that binds cereblon to induce ubiquitination and degradation of transcription factors Ikaros (IKZF1) and Aiolos (IKZF3), altering cytokine production and exerting direct and immune‑mediated antitumor effects. Preclinical and clinical studies suggest mechanistic synergy between ibrutinib and lenalidomide via suppression of IRF4/NF‑κB signaling and enhancement of antitumor immune activity in diffuse large B‑cell lymphoma and indolent lymphomas. This doublet has most often been studied as part of the triplet with rituximab (iR2/RLI), but the core two‑drug combination is the pharmacologic basis of the regimen.[4][2][3][1]
